Information gathering
Natural Healthcare is a natural medicine consultancy practice. In order for us to do our work we need to gather information about your current, past and family health, it is routine that we request a thorough and detailed health history, we will not collect information that is not reasonably necessary for us to do our job. This information is gathered by you completing our forms, (completed prior to your first appointment) and verbally during your scheduled appointments. With your consent we may also request information from other healthcare providers including test results. Your information will not be shared without your prior written consent, unless we obliged to do so by law.
Record keeping
Records are kept in hard copy and/or electronically. Hard copies are kept in a lockable filing cabinet and are only accessed by your practitioner or an authorised member of staff. Electronic records are password and authenticator protected and are only accessible by your practitioner.
Who has access to my records?
Natural Healthcare (Karen Gay) employs a reception service (Time Well Spent) to help with practice management, they have password protected access to your contact details only; They are bound by the privacy act to not disclose any information that they may encounter as a part of their job.
Student observation and mentoring
On occasion Karen Gay may have a student sitting in with her, they are also bound by the privacy laws and can only observe with your consent. From time to time we may request to use your case as a part of mentoring, this is done only with your prior consent. No information that would disclose your identify will be shared.
Sharing information electronically
We will electronically send you your blank new patient forms. You can either email your completed forms and results back to us or print off a hard copy and return them by post or drop them off at the clinic directly. We do not take responsibility for how you handle or share your information.
Sharing information with overseas entities
As a part of the privacy policy regulations we need to declare if and how information may be shared with overseas entities. We will never share your information with oversea entities without your prior written consent.
Accessing your personal information
You may at any time request a copy of your history and or results. This request must be made in writing. We will provide you with this information in a reasonable amount of time. A report writing fee may apply some cases, the fee will reflect the amount of time taken. No fee will ever be charged for you requesting your information.
Minors
A minor is a child under the age of 16. Any child under the age of 16 must be accompanied by a parent or legal guardian at all times.
